Documents together with the signed application form:

1. Primary One Registration Form
    
2. Applicant child’s Birth Certificate
   1. 1 Hong Kong Birth Certificate (original and photocopy); or
   2. 2 Non local Birth Certificate and together with the document(s) permitting him/her to stay in Hong Kong (original and photocopy)
       
3. Student’s photo
    
4. The parent/guardian’s Identity Card, Passport or other documents (if the parent/guardian cannot come in person, his or her representative should present his/her Identity Card, a photocopy of the parent / guardian’s identity documents and the letter of authorization)
    
5. Documents of residential address. 
   The document including but not limited to stamped tenancy agreement, demand note for rates, public housing tenancy agreement or water/electricity/town gas/ residential telephone bills and a photo of the document. 
   (Please note that the address on a bank letter, court summons and tax bill may be a correspondence address are ot a residential address.  Therefore, such documents have not been accepted as proof of residential address by the Education Bureau all along.)
    
6. Completed Primary One Registration Form (download the form from the school website).
