收集個人資料聲明

(一)本校會用本表格所載的個人資料，作以下其中一種或多種用途：

1. 辦理申請入學事宜

2. 處理入學所需事務

3. 提供教育服務

(二)本表格所蒐集的個人資料，或會向獲授權處理有關資料的學校 / 組別 /部門 / 機構披露，作上文所述用途。

(三)在作出申請時提供資料純屬自願，如沒有提供足夠和正確的資料，其申請或無法處理。

(四)資料當事人可要求查閱及/或改正該表格上的個人資料。

Personal Information Collection Statement

(1) Our school will use the personal data provided in this form for one or more of the following purposes:

1. Apply for admission

2. Handle the affairs required for admission

3. Providing educational services

(2) The personal data collected in this form may be disclosed to schools/groups/departments/organizations authorized to process the data for the purposes mentioned above.

(3) The provision of information when making an application is purely voluntary. In the event that sufficient and correct information is not provided, the application may not be processed.

(4) The data subject may request for access and/or correction of the personal data on the form.
